Requesting an information package?  If you would like to receive an information packet by postal mail please complete the form below.  I invite you to call me in order to learn more about our program and it's my pleasure to answer any questions you may have.  Upon request I'll send a packet including an information letter, sample brochures, and an agreement form for you to view and share with your organization.  Your packet will be mailed within 24 hours. Please allow 3-5 days for delivery.
